BOXING GYM WAIVER & RELEASE OF LIABILITY

Liability Waiver

I acknowledge and understand that participation in boxing and related physical activities at Fight Club 1947 involves inherent risks, including but not limited to physical injury, illness, or, in rare cases, death.

I voluntarily accept full responsibility for any injuries or damages that may occur as a result of my participation in training, sparring, fitness classes, or use of the gym’s facilities and equipment.

I release, waive, and discharge Fight Club 1947, its owners, trainers, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from my involvement.

I certify that I am in good health and physically capable of participating in boxing and fitness activities. I understand it is my responsibility to consult a physician before beginning any exercise program.

I agree to comply with all gym rules and instructions provided by the staff. Failure to do so may result in suspension or cancellation of my membership.

FOR MINORS (Under 18 Years Old)

As the parent/legal guardian of the minor listed below, I give permission for my child to participate in all boxing and gym-related activities at Fight Club 1947. I understand the risks involved and accept full responsibility for any injury or incident that may occur.

I hereby release Fight Club 1947 and its staff from any liability or claim arising from my child's participation.
